We encourage everyone to submit a photo or video of your successful return to daily life. We can work with images from all sources, including cameras, a cell phone, camcorder, or a selfie stick.
Once approved and verified by you, your testimonial will be posted on The ONS Experience within two business days.
The ONS Experience submission forms are not saved into an account and does not serve as login information for any other part of ONSMD.COM, so you will not be able to return to the page once you've clicked out. If you are unable to submit your testimonial on your first attempt, we suggest you create a draft in a separate document to later cut and paste into the submission form at your convenience.
Patient's must be 18 years or older to submit a testimonial to The ONS Experience. A parent or legal guardian can submit a story on their behalf, however.

Your "The ONS Experience" story has no expiration date and will live for many generations of patients to see. Also, we will continue to collect and post stories to The ONS Experience site as they come along for all to see.
Patients can submit multiple photos. Once approved, your images will be posted as space permits along with your story.
If you don't see your condition or treatment listed, the Orthopaedic & Neurosurgery Specialists team will match to the appropriate term and category.
In addition to The ONS Experience site, your story may have a presence on the official Orhtopaedic & Neurosurgery site (www.onsmd.com) and may be listed in their respective condition pages and by physician or therapist. Other possibilities include posters and promotional materials that will be displayed onsite at our Orthopaedic & Neurosurgery Specialists locations.
Stories must be at least a paragraph long (4-5 sentences). Of course, you are more than welcome to share as much as you would like.
